Introducing Touchpoints - A Letter From the Administrator
Dear Resident and Family Members
As the Administrator, I would like to welcome you to Touchpoints at Farmington. At our center, caring for the whole person is the philosophy that is reflected in the attitude and performance of every member of our team. Additionally, we strive to exceed your expectations. In doing so, we understand that we must partner and participate with you in your plan of care. Communication along the way will be extremely important. This will include a brief meeting within a few days of your arrival, Interdisciplinary Care Conferences, Discharge Planning Meetings and consistent communication with your caregivers along the way.
We offer a variety of programs specially designed to meet the individual needs and goals of each of our short-term patients and long-term care residents. Our Touchpoints Rehabilitation Program is designed for those who may require a post-acute stay for orthopedic, surgical or medical recovery, intravenous (IV) therapy, wound care, hospice services and pain management. We are exceptionally proud of our newest endeavor, our Congestive Heart Failure (CHF) Recovery Program, which includes collaboration with medical, physical, educational and psychosocial supports to return people to home and reduce and/or eliminate re-hospitalization. We pride ourselves on offering skilled Physical, Occupational and Speech Therapy services available seven days per week to those in need in our spacious, state-of-the-art rehabilitation gym.
We are also exceptionally proud of our Dementia Program which is a secure environment in which residents are cared for by highly-trained professionals under the direction of a full-time Program Director. Residents are treated with dignity and respect and are welcome to participate fully in a wide variety of therapeutic recreation activities to meet their physical, cognitive and emotional well-being.
