Touchpoints at Farmington pulmonary rehabilitation program is designed to bring continuity in pulmonary care from the acute hospital setting into the skilled nursing and rehab care setting. Board certified pulmonologists Dr. John Rodgers and Dr. William Preskenis provide pulmonary care, sleep medicine and specialty consultation to the residents of the Touchpoints at Farmington community. In addition an experienced, registered Respiratory Therapist rounds within the facility, providing assessment and care to those with respiratory and pulmonary disease processes.
This partnership will give our residents access to expert, individualized and clinically outstanding pulmonary care and sleep medicine in the short term rehab setting.
At Touchpoints, we recognize that proper respiratory function directly impacts the quality of life and ability to conduct normal daily activities with ease. Our Pulmonary Rehabilitation Program features care and therapy designed to treat and manage respiratory illnesses such as:
- COPD (Chronic Obstructive Pulmonary Disorder)
- Cystic Fibrosis

- Primary Pulmonary Hypertension

Our Pulmonary Program includes:
- Initial and ongoing assessments from physical, occupational, respiratory and speech therapists up to 7 days per week.
- Rehabilitation with respiratory and physical therapists
- Board-certified pulmonologists
- Spirometry screening pre and post bronchodilator
- Six-minute walk / activity testing
- VEST Therapy
- Exercise and endurance training
- Oxygen titration
- Extensive patient and family education
- Case Management and discharge planning services
Addressing frequent hospitalizations.
The program also addresses repeat hospitalizations by applying integrated pulmonary care and sleep medicine consulting to many diagnoses, including COPD and congestive heart failure. For patients will multiple hospitalizations the pulmonology team will address their general state of health and wellness and improve their daily functionality. This will get them back home where they want to be and slow the tide of re-hospitalizations. It is truly forward looking.
Sleep Medicine: In-house Sleep Studies
At Touchpoints Rehab, our Pulmonology collaborative program offers sleep medicine consultation and sleep studies. Patients are able to receive treatment for pulmonary disease and have a sleep study, analyzed by a sleep specialist, all without leaving the skilled nursing center.
Touchpoints Rehab uses the ARES Watermark device for Sleep Studies and is one of only a few skilled nursing centers to provide this service in the State of Connecticut.
An overnight sleep study, known as a polysomnogram, under the supervision of a sleep study specialist is considered the gold standard for diagnosing sleep disordered breathing. Patients who report snoring, excessive daytime sleepiness or fatigue, and poor sleep quality should have their sleep assessed by a formal sleep study.
Sleep disordered breathing broadly comprises obstructive sleep apnea, when upper airway instability causes mechanical obstruction to breathing; and central sleep apnea, characterized by an absence of ventilatory effort.
About the pulmonary team.
Dr. Rodgers and Dr. Preskenis (pictured above, L to R) are affiliated with Prime Healthcare and have been in practice for over 20 years respectively. Dr. John Rodgers received his medical degree from the University of Connecticut School of Medicine and is board certified in pulmonary medicine. Dr. William Preskenis received his medical degree from Saint Louis University School of Medicine and is board certified in pulmonary and sleep medicine.

Touchpoints at Farmington is an innovative skilled nursing center that specializes in Congestive Heart Failure (CHF) and Pulmonary Rehabilitation and works closely with and accepts referrals from all area hospitals as well as placements state and regionwide. Touchpoints is well versed in a vast array of disease processes which include diabetes management, wound management, COPD, hip and joint replacement, and many others. The center also provides outpatient therapy, long term care and behavioral health services.
