Dear Resident and Family Members:
I would like to welcome you to Touchpoints at Farmington and thanks for visiting our page! At our center, caring for the whole person is integral to our philosophy. It is reflected in the attitude and performance of every member of our team everyday.
We strive to exceed your expectations and in doing so, we understand that we must partner and participate with you in your plan of care. Partnership and communication will include an introductory and planning meeting within a few days of your arrival, Interdisciplinary Care Conferences, Discharge Planning Meetings and consistent communication with your caregivers all along the way.
We are proud members of three major post acute care networks: Trinity Health of New England - SOHO Health Integrated Network, the Hartford HealthCare Integrated Health Network and UConn Health Preferred Provider Network. As such, we are partnered with both systems and physician medical staff to provide high quality care to our mutual patients.
We offer a variety of programs specially designed to meet the individual needs and goals of each of our short-term patients and long-term care residents. We are exceptionally proud of our signature Congestive Heart Failure (CHF) Recovery Program, which includes collaboration with medical, physical, educational and psychosocial supports to return people to home and reduce and/or eliminate re-hospitalization. Our Touchpoints Rehabilitation Program is designed for those who may require a post-acute stay for orthopedic, surgical or medical recovery, intravenous (IV) therapy, wound care, hospice services and pain management.
At Touchpoints, our goal is to have all staff engage in resident specific care. We recognize that all of our residents have different needs and require specific care to fulfill those needs. We hope to accomplish this by partnering with our residents and family members so they may achieve all of their goals. We find that the communication between staff and resident is crucial from the very beginning so we may be able to meet and exceed all expectations. Whether it be for short-term care or our Memory Care unit we are able to provide the highest level of customer service.
We strive to treat the “whole person” during their entire duration. We make every attempt to accommodate all areas of need and do so by embracing this concept. We have extremely talented and compassionate individuals that truly care about serving others. At Touchpoints we understand how difficult it is for families to make decisions for their loved ones. We want to put your concerns to rest as we are confident that we can meet all of our resident’s physical, emotional and psychosocial needs.
